YSBA Awards of Excellence  Nomination Form 

Each season the YSBA shall present Rep and Select awards for some or all (at the discretion of the YSBA Board of Directors) of the following categories:

Association of the Year
Doug Sutton (Volunteer of the Year)
Junior Umpire of the Year
Senior Umpire of the Year
REP Player of Year 13U and below
REP Pitcher of Year 13U and below
REP Coach of the Year 13U and below
REP Player of Year 14U and above
Rep Pitcher of Year 14U and above
REP Coach of the Year 14U and above
REP Team of the Year
Select Player of the Year
Select Coach of the Year
Select Team of the Year
House League Coach of the Year 13U and below
House League Player of the Year 13U and below
House League Coach of the Year 14U and above
House League Player of the Year 14U and above
 
- All nominations must be submitted electronically via the YSBA website, by no later than October 1st of the current season.
- Only nominations that are approved by the applicable Local president will be considered.
- Only the YSBA award winners shall be approved for submission to Baseball Ontario as the YSBA nominees to be considered for the annual OBA Awards.
- The YSBA Awards of Excellence winners will be announced at the YSBA AGM (typically in October of each year).
- Award winners and a guest will be invited to the YSBA AGM

Process:
  2) Review and complete request form. (Digital Form PDF)
  3) Submit to the via email to the YSBA Policy & Procedures committee chair for approval
     ** ensure proof of certification is attached
  4) YSBA - CPPC will review and approve/reject based on OBA established first aid rule (see above)
  5) If approved a Trainer number will be issued, this code is to be listed on the OBA roster
